How Subcision Gets Rid of Acne Scars

Subcision is a medical procedure with which a doctor or dermatologist may try to reduce the appearance of acne scars on the skin. Subcision may take several procedures to get rid of acne scars and does not work for every person. It is only used for cases in which the acne scars are pitted, which includes ice pick scars, rolling scars, and box car scars.

When severe acne breaks out on the skin’s surface, there is a much deeper infection that occurs within the pore. Deep in the pores, the structures and tissues surrounding hair follicles and sweat glands can become severely damaged. In response to this damage, the body naturally sends out collagen proteins to replace the broken tissues. As collagen fills the damaged tissue, the fibers in the collagen connect.

Because the damage occurs deep in the pore, scar tissues on the top of the wound are drawn down into the pore by the underlying scar tissue. This creates the pock marks associated with most acne scarring.

Additionally, a surface scar appears because the fibers in this new collagen are not connected in basket weave pattern like normal surrounding skin, making the scar appear different than other skin. To get rid of the acne scars, doctors typically address both the level of the scar tissue in comparison with the surrounding skin and the color or physical appearance of the scar.

During the subcision procedure, the doctor will insert a small device, usually a special needle, to create a void beneath the upper layer of the acne scar. (Although little to no pain is incurred during the procedure thanks to local anesthesia, some pain may be felt during healing.) When the skin begins to heal, this void fills and the upper layer of the acne scar can become level with the skin’s surface over them.


How Subcision Works Get Rid of Acne Scars


Doctors may use subcision alone to get rid of acne scars that are not deeply drawn into the pores of the skin. When the doctor uses a needle to disconnect the upper layers of scar tissue with the deeper parts of the pore, a void is created in the middle. The body naturally fills this void with new tissue (collagen proteins) and the upper layer of the scar tissue is raised as the void heals.

In cases of deeper acne scarring, it may take an injection to help get rid of the acne scar. Doctors have at their disposal a number of substances known as “dermal fillers” that can quickly do what the body would naturally do. Dermal fillers quickly fill the void left by subcision and can further raise the level of the scar. They also allow doctors greater control over the collagen filling process.

Dermal fillers that help to get rid of acne scars may be comprised of silicone, fat, or collagens. They are almost exclusively used following a subcision procedure to prevent any negative effects on the appearance of the skin. (Using only a filler can leave the surface uneven.)
